Summary

Provides prescribed medical treatment and personal care and services to persons with disabilities in residential homes and/or vocational centers at a single business location. Relies on knowledge, experience, and judgment to accomplish tasks in combination with clinical supervision from the registered nurse and direction from the Registered Nurse and/or Administrator. Requires a graduation from an accredited nursing program and an unencumbered nursing license.

Essential Job Related Responsibilities

Assists with Health and Safety Procedures- Instructs Infection Control Conrol and other classes as directed by the RN and/or Administrator.. Also participates in employee orientation and in-service training/educational programs. Seeks understanding of and follows company personnel policy, regulations, safety, bloodborne pathogens & communicable diseases. Reports accidents, hazards, and needed repairs. Understands, teaches and utilizes proper body mechanics while assisting with behavioral intervention techniques, dependent/independent transfers and positioning of people supported regardless of individual weight distribution. Practices universal medical precautions by understanding and utilizing personal protective and safety equipment. Ensures personal protective equipment is readily available in the group homes and vans during the monthly nursing house assessments. Maintains current CPR/First Aide Certification through the American Heart Association.

Assists Professional Staff with Various Tasks- Assists the RN in monitoring Medication Tech’s (if allowed by state law) to ensure compliance with RHA policy and standards, as well as compliance with state and federal regulations. Assists RN and professional staff in delivering quality nursing services to people supported. Consults with RN on nursing problems and interpretation of company and unit policy to ensure the person’s needs are met. Assists the physician and the RN in preparation of clinics, assists during medical and dental clinics, also implements new physician orders following clinic. Assists the RN in re-enforcing medication administration techniques taught in the Medication Administration Course (if allowed by State law). Instructs medication administration certified Direct Support employees in administering new or changed medication orders and monitors their performance. Serves as an active member of the interdiscplinary team. Works closely with QP and other clinical/professional staff to maintain communication and provide feedback, standardize procedures, expedite habilitation plan implementation and to improve efficiency of direct support staff. Schedules and participates in person supported and employee physical examinations. Contacts physician and hospital to arrange for further medical treatment, when needed. Schedules diagnostic tests and other physician consult appointments as ordered by the primary care physician and as indicated by the health needs of the people supported.

Performs LPN Responsibilities- Provides nursing services after hours and on weekends and holidays as scheduled and according to the company’s On Call Policy. Completes an After House On-Call log and submits to the Nursing Supervisor and Business Manager. Attends mandatory meetings and training sessions as required and scheduled by supervisor, the Administrator or Corporate Officers. Maintains harmony among co-workers, emphasizes professionalism and serves as role model for direct support staff. Incumbents in this position must be able to perform the essential functions on any shift and at any one of the company's work sites.

 

Required Education/Experience

Education- Graduation from an accredited practical nursing program

Experience- Prefer six months to one year clinical experience in the IDD profession

 

Required Licenses/Certifications

Valid driver’s license

Mathematical -Ability to calculate figures and amounts such as addition, multiplication, division, percentages.

 

Reasoning- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

 

Essential Physical and Sensory Requirements-With or without the aid of mechanical devices

Regular required to lift 20 lbs. Must be able to lift a minimum of 40 lbs. Must be able to pull minimum of 25 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear.
