Job Description

Provides support for individuals and/or groups of persons with disabilities in the community.  Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports. Implements services and supports to address the person’s goals and desired outcomes as identified through each person’s Centered Plan. Seek to understand the individuals in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Relies on direction and supervision to accomplish the job.  Requires a high school diploma or GED with some experience with the population supported preferred. 

Job Requirements

Assists with Data Collection and Reporting- Ensures collection & documentation of required data.  Writes in logs, incident reports, program data sheets, etc. noting progress/events. Monitors and reports on the health and wellness of people supported


Required Education/Experience

Education- Must have a High school diploma or (GED)

Experience- Prefer experience working with IDD individuals


Required Licenses/Certifications

Maintain CPR and first aid certification , Med Tech and NCI certifications and others that may be specified and/or required by future company regulations

Valid driver’s license

Essential Physical and Sensory Requirements-With or without the aid of mechanical devices

Regularly required to lift 30 lbs. Must be able to lift a minimum of 70 lbs. Must be able to pull minimum of 50lbs.    Must be able to squat, kneel, crawl, crouch, climb, and stoop.  Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.  Required to regularly stand and walk.  Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting.  Vision requirements include close vision, distance vision, and peripheral vision.
