Job Description

Seeking individuals with a strong case management and/or direct care experience with adults with mental health diagnoses. Seeking either Certified Peer Support Specialists or Associate Professionals (AP). This position requires an individual who is organized, self-motivated, has strong documentation skills, and is comfortable with working with individuals in their homes. You will be part of the Transitional Management Team (TMS), which is a team that works with individuals that are part of the Transition to Community Living Initiative (TCLI). Our clients have SPMIs (Severe and Persistent Mental Illnesses) and have transitioned from group homes and assisted living facilities and are now living independently in the community. They are wrapped with both community and outpatient mental health services including TMS. As a TMS staff you will be helping clients develop, implement, and master the life and interpersonal skills necessary to help them maintain in an independent living environment. Working with the MCO and other providers is an integral part of the normal routine, so professionalism is a must. This position requires you to spend the bulk of your time in the field working directly with clients.

Job Requirements

Candidates for the Behavioral Health Specialist role are considered “Qualified Professionals" based on the following criteria:


Education:

  • Bachelor Degree (Not Human Services field) & 4+ years full-time experience with population served OR
  • Bachelor Degree (in Human Services field) & 2+ years full-time experience with population served OR
  • Master’s Degree or Higher & 1+ years full-time experience with population served.
  • License, Provisional License, Certificate, Registration, Permit issued by governing board regulating human service profession.

Definitions:
  • Human Service Degrees Include: Social Service, Sociology, Psychology, or other Human Service Degrees.
  • Populations Served Include: Mentally Ill-Child, Mentally Ill- Adult, Substance Abuse-Adult, Substance Abuse-Child

Additional requirements of the Behavioral Health Specialist include:

  • Must meet Qualified Professional requirements
  • Must meet the education, experience, licensure/certification requirements specified by the service definition of the service being provided
  • Valid driver’s license and automotive insurance required
  • CPR, first aid, NCI and/or relevant certification, a plus
